Slovenia Schengen Visa Application Requirements

Luxembourg Visa Application Requirements

3. Copy of the data page of the passport together with all pages having visas and stamps

6. Proof of travel. Copy of the roundtrip airline ticket reservation with Passenger name & Registration Number (or reservation number). Do not pay for/buy the ticket until a visa has been granted. 7. Proof of Travel Medical Insurance

Sweden Schengen Visa Application Requirements

Sweden Visa Application Requirements

1. Cover Letter must be printed from the Application Portal and signed by the applicant. It has the receipt number which serves as the applicant’s proof of payment.

3. One (1) passport sized photo with white background and not older than 6 months. Edited photos are not accepted. Paste your photo (do not staple) on the upper right corner of the Cover Letter

4. Photocopy of Original Passport – biopage and used pages (with visas and stamps) Original passport should be submitted along with the application. It should be valid for at least 3 months after departure from Schengen Area. Valid passport must have at least 2 unused pages (if approved, the visa will be affixed on said page/s).

5. If visiting a relative, submit proof of relationship photocopies of NSO Birth Certificates/Marriage Certificates

6. If visiting a friend, submit his/her photocopies of passport with stamps including biopage to prove time spent together. If both parties have spent time together outside the Philippines, both parties’ passports with stamps should be submitted.

7. Original Copy of Invitation form no. 241011 – completely filled out and signed by the inviting person This form can be downloaded and printed from the Embassy’s website.

8. Original Copy of Civic registration certificate (personbevis) of the inviting person

9. Photocopy of proof of sufficient funds of the inviting person/sponsor – three most recent payslips, pension statement, statement of account for the past three months, or

Original Bank Certificate of the Applicant should be submitted if the inviting person will not financially support your stay. Applicant is required to have SEK 450/day. The requirement can be lessen if he/she already paid the lodging or boarding in advance or will stay with the inviting person in Sweden.

10. Photocopy of Roundtrip Airline Ticket Reservation – Do not pay for or buy the ticket until a visa has been granted. Note: If the application is approved, the visa will be issued according to your roundtrip flight reservation. Read more about flight itinerary for visa application.

11. Photocopy of Travel Medical Insurance – can be bought from any insurance companies; should be valid in all Schengen States and cover the entire period of the person’s intended stay or transit. The minimum coverage shall be EUR 30,000/ PHP 2.5M/USD 50,000.

12. Proof of accommodation: Hotel confirmation, hotel reservations or hotel voucher. You need to show hotel reservation booking for all cities you are visiting on your trip.

 

Additional Mandatory Requirements for Minors (under the age of 18)

13. Photocopy of NSO Certified Birth Certificate of the applicant – original copy to be presented.

14. If traveling alone: Original letter of consent signed by both parents and photocopies of passports/valid Ids of parents with signature

15. If traveling with only one of the parents: Original letter of consent signed by the other parent and photocopy of valid ID w/ signature.

16. If only one parent has the sole custody of the child: Photocopy of Court Decision awarding custody to that one parent must be submitted.

Germany Schengen Visa Application Requirements

Germany Visa Application Requirements

1.  One fully completed application form signed by your own hand and completed by you (online). Declaration according to § 54 AufenthaltsG, fully completed and signed by your own hand. You can see the Schengen Visa Application Form here.

2. Your original passport, which must be valid for at least another 3 months upon the conclusion of the travel and must contain two empty pages and one Xerox copy of the passport, and further valid / invalid passports

3. One current passport picture

4. Details regarding the travel destination and travel route. Details regarding the means of travel for the outward – and return journey. Details regarding the flight reservation, no flight ticket needed. Read more about flight itinerary for visa application.

5. Hotel reservation of the hotel/guest house as proof that the accommodation has already been paid for

6. The financial coverage of the cost of your travel and stay. Statements of your account(s) for the previous six months, with a confirmation by the bank.

7. Travel health insurance, which is valid for all Schengen-States and the full duration of stay for which the visa is applied for, with a minimum coverage sum of 30,000 Euros. Insurances from your country must be accredited and given in the original with a Xerox, German insurances are accepted as a Xerox, Fax or scan.

8. If you are employed:

a.  Proof of your employment, i.e. most recent income tax return, certificate of employment (including your employer’s full name, complete address and telephone number with area code details regarding your position or professional designation, income, duration of the working relationship)
b.  letter of approved leave of absence signed by your employer

If you are self-employed:

a. proof of registration and commercial activity of your company in the Philippines, e.g. income tax returns, bank certificates, documents of the company accounts or other.

If you are a pupil/student:

a. School certificate / University certificate
b. proof of enrollment (for students)
c. if applicable: letter of exemption from studies

If you own real estate property

a. e.g. original land title, deed of sale

9. Additional documents for children below age 18:

a. Personal appearance of both parents during submission of the visa application with valid ID (passport or driver’s license)
b. Original Birth Certificate of the child

If traveling without both parents:

a. Declaration of consent by the legal guardian(s), signed in front of a German consular officer
b. Passports of the legal guardian(s) (original and Xerox copy of the data page) or
Proof of legal custody (for minors with parents whose marriage was dissolved/ annulled) or
Death certificate of the other parent
c. Travel Permit, issued by the Department of Social Welfare and Development (DSWD); please contact the DSWD for further information. The according proof should be carried along and submitted to the border control.

*If the person(s) having care and custody of the child is resident abroad, a declaration of consent, given before the responsible German (honorary-) Consul must be submitted. If the person(s) having care and custody of the child live(s) in Germany, this declaration must be given before a German Notary public or the responsible aliens authority (Ausländerbehörde).

Iceland Schengen Visa Application Requirements

Iceland Visa Application Requirements

1.Cover letter and application form. It must be signed. You can see the Schengen Visa Application Form here.
Note: For children under 18 years old, both parents must sign the application form. Passport or ID copies of the father and mother have to be attached.

2. One (1) photograph. Note: Passport size with white or plain light color background, frontal view (3,5 x 4,5 cm); (not older than 6 months).

3. Passport (both new and old passport(s) if any). Please make sure that your passport is valid for at least three months beyond your intended stay in Schengen, and that the passport has at least two blank pages for a visa sticker.

4. Travel Plan (In Norwegian, Danish or English) stating:
a. Arrival and departure dates.
b. If you plan to travel in and out of Schengen, you must state in the application form that you want several entries. It is particularly important for seafarers to remember this.
c. Evidence of hotel booking.
d. If you plan to visit several Schengen countries in the course of one trip, you must submit your application to the embassy or consulate of the country that is your main destination.
e. If the visit is arranged by a travel agency: all documentation supplied that the trip has been purchased as a package which includes, at a minimum, flight reservation and hotel reservations.

5. Employment / Student letter:
a. If the applicant is employed: Support letter from the employer (in original on company stationary in English or with sworn English translation), stating how long the applicant has worked in the company, the position of the applicant and pay slips for the last three months.
b. If the applicant is an employer or owner of a private company: company registration with translations in English.
c. If the applicant is retired: proof of pension (letter from Social Security Department or bank account statement translated into English)
d. If the applicant is a student: Proof of enrollment.

6. Updated bank account statement of the applicant, in original for the last 3 months. Signed and stamped by bank. Please note that a letter from the bank stating only the balance is not valid. International credit cards, saving books, proof of retirement benefits or deeds of real state property. For students: Parental warranty letter + Financial means of the parents or person who will pay the expenses (with copy of their passport or ID attached).

7. If the applicant is under 18, you must submit:
a. If you are travelling alone or with only one of your parents: An original letter of consent issued by the local police station signed by both parents and translated into English. (copy of the parents’ passport)
b. If the parent you are travelling with has single custody: An original letter of consent and translated into English, and a copy of certificate of single custody

8. Compulsory travel medical insurance: Must cover all Schengen countries. Minimum coverage required is 30,000 EUR.

9. Flight reservation or other proof of intended transport like train reservation or car rental. It is not necessary to buy the ticket until the visa has been approved.

10. Signed invitation letter from Iceland.

11. Original Birth Registration issued by the Civil Registry Office.

12. Original and a photocopy of the Marriage certificate, if married.

Switzerland Schengen Visa Application Requirements

Switzerland Visa Application Requirements

1. One (1) visa application form, which must be completely and properly filled-out and signed by the applicant (write in block letters and use black or blue ink only). You can see the Schengen Visa Application Form here.

2. Two (2) identical, clear and coloured passport-sized photos (With white background; Must be taken within the last six months; Please do not staple the photos on the form).

3. Valid passport (Must be valid at least 3 months from the end of the approved duration stay; Must contain at least 2 blank visa pages.)

4. Photocopy of valid passport as well as old passports (Inside front cover containing holder’s personal information and photo; Inside back cover containing „Important reminders“, signature and other passport information.)

5. Photocopy of all visas (valid and expired)

6. Computer printout of confirmed roundtrip flight reservation. (Do not yet buy a flight ticket. The Embassy cannot be held responsible for any financial losses in case you have to return a ticket.) Read more about flight itinerary for visa application.

7. Detailed day-to-day itinerary of planned trip/schedule of activities. You can also show train reservations if applicable.

8. Proof of accommodation: booking confirmation or hotel reservation containing the complete name and contact information of the hotel with hotel voucher

9. If applicable, valid visa(s) for the country(-ies) of destination after leaving the Schengen territory based on the planned itinerary.

10. International travel insurance (May be purchased locally or in Switzerland; must have a coverage of at least EUR 30,000/ USD 50,000 to cover expenses for personal accident, urgent medical treatment, urgent admission in a hospital or urgent repatriation on medical grounds; with coverage including ALL Schengen member states). The validity period of the insurance coverage must be for the entire duration of the intended stay including entry and exit day of the Schengen area.

11. If employed: certificate of employment with leave of absence from the local company or organisation showing position, date of hiring, salary as well as date of return to work.

12. If self-employed: proof of business (DTI/SEC business registration, income tax return, financial statements)

13. If still studying: proof of enrolment, leave of absence from school (if traveling while classes are ongoing)

14. If traveling with spouse and/or children: marriage contract and birth certificate of the children

15. Proof of financial resources of the applicant: latest bank statements showing cash flow from the last 6 months, bank books, latest international credit card statements (last 6 months), Income tax return

16. For spouses or children (below 21 years old) of Swiss / EU / EFTA nationals, the following must also be submitted: photocopy of the marriage contract (spouse), birth certificate (child), family book or any other document proving relationship, which must be issued by a Schengen / EU / EFTA member country. A copy of the family member’s passport and/or ID card must also be provided. You are NO longer required to submit the documents enumerated below. In some cases, however, the Embassy still reserves the right to request for additional documentation.

Portugal Visa Application Requirements

1. Online Visa Application Form completely filled and signed by the applicant (Date on Online Visa Application form should be the same date as the appointment for visa submission). You can see the Schengen Visa Application Form here.

2. One Recent (not older than six months) Passport-Size Photographs in color with white background. (3.5cm / 4.5cm) (Do not Staple)

3. Personal Covering Letter from the applicant introducing himself/herself, explaining the purpose, duration & itinerary of the visit in brief. The cover letter should also mention the name and relation of any accompanying travelers with their passport numbers and passport copy. If the accompanying travelers already possess a valid Schengen Visa, please enclose a photocopy of the same and if applying through another Embassy/Consulate, please provide the details.

4. Original Proof of Occupation – Employment Covering Letter (if employed) from the company mentioning the designation and current profile of the applicant, purpose of the visit, itinerary, name & contacts of the company in Portugal with complete and full address and contacts of host/hotel accommodation in Portugal.

5. Reserved onward ticket or Roundtrip flight reservation of travellers. You do not need a fully paid flight ticket. Read more about flight itinerary for visa application.

6. Internal Travel Tickets/Reservations if traveling to more than one country within Schengen area. This can be flight reservation or train reservation.

7. Overseas travel medical insurance valid for all Schengen-countries .The insurance has to cover the applicant for at least 30,000 Euros or equivalent, for all risks e.g. accident, illness, medical emergency evacuation etc. The policy has to clearly specify the period of validity and has to cover the entire duration of the trip including the date of arrival & departure India

8. Proof of Accommodation (confirmation of Hotel reservation or Apartment booking).

9. Personal Proof of sufficient funds to cover all expenses while in Portugal:
a. Bank Statements for the last 6 months
b. Copy IT Returns Form-V for the last 3 years
c. Copy Salary Slips for the last 3 months (if working).

10. Sponsorship Proof:
If the applicant is sponsored by another person, an original Sponsorship Letter is required and must be notarized. The Sponsorship Letter should be Notarized/Attested by a Notary/Advocate if in the E.U. or from the respective Portugal Consulate/Embassy in other countries.

11. Minors below 18 years traveling alone must submit the written consent (NOC) from both their parents/guardian on:
a. Stamp Paper signed by both Parents / Guardian mentioning Financial Sponsorship for Travel and must be Notarized.
b. If the minor is traveling with only one Parent or Guardian, the same NOC will be required from the other Parent / Guardian not traveling with them.

12. Declaration form granting SEF (Foreign and borders office) permission for consulting criminal records in Portugal (not required for minors below 16 years).

13. The Passport should be valid for at least three months beyond the intended duration of stay in the Schengen territory and with at least two blank pages to affix the visa. No visa will be issued on the additional page of the passport.
a. Copy of the Bio-Pages (First and Last page) of the Passport including the copy of the page stamped by the Passport Office must be enclosed.
b. Schengen Visa copies if issued in the last 3 years

Greece Visa Application Requirements

1. One (1) duly filled (typewritten) application form.  You can see the Schengen Visa Application Form here.

2. One (1) recent passport-sized photo with white background pasted on the application form.

3. Passport with a minimum remaining validity of six (6) months after the starting date of the
travel, and at least two (2) adjacent free pages and no existing valid Schengen visas.

4. All previous passports. Affidavit of loss for any previous passports lost, stolen or destroyed.

5. Flight reservation (purchased ticket is not required, reservations is fine) including flights within Schengen countries. You can also show train reservation if applicable. Read more about flight itinerary for visa application.

6. Confirmed hotel reservation for the entire duration of your stay in the Schengen area.

7. Travel health insurance from an internationally recognized insurance company, for the entire duration of your stay in the Schengen area, valid in all Member States and with a minimum coverage of €30,000.

8. Proof of Financial Means (all documents required)
a. Formal bank certificate addressed to the Consular Section, indicating the applicant’s bank account remaining balance and average daily balance for the past six (6) months, accompanied by past six (6) months account statement and original bank book.
b. Latest income tax return form (ITR).
c. Property titles, if applicable.

9. Proof of Employment Status
a. For employees: formal certificate of employment addressed to the Consular Section stating salary level and authorizing leave of absence for the duration of the travel.
b. For self-employed: latest business permit, company’s registration and incorporation document (GIS), and company ITR.
c. For University students: recent certificate of enrolment, and formal letter authorizing the student’s absence outside regular holidays.

10. If invited/sponsored:
a. Dully signed recent original invitation letter from the inviting person or company addressed to the Consular Section, guaranteeing repatriation and coverage of travel, accommodation and any medical expenses, accompanied by a photocopy of the signatory’s id/passport.
b. If invited by Greek: solemn declaration (“Υπεύθυνη Δήλωση”) with authenticated signature and same content as above. If invited by Portuguese: “Termo de Responsabilidade”.

a. Joint notarized affidavit of both parents/legal guardians giving their consent for the travel, accompanied by photocopies of their passports.
b. Recent original copy of birth certificate
